New Patient Registration

To register with the Practice you will need to submit your medical card with part A signed and completed. If you do not have a medical card ask the receptionist for a GMS1 form and complete this. It is important that your NHS number is recorded on the form.

On completion of the card or GMS1 form you will be expected to attend an appointment for a New Patient health check with the Practice nurse at the surgery. This is a statutory requirement of the practice and failure to comply with this will result in non-registration.

Please ask at reception if you need a diagram of the practice boundary.

Change of Address

If you change your address, name or telephone number, please notify our receptionist so that your records held at the surgery can be updated.
